Leaked Albums....how??

I've always wanted to know how these albums get "leaked?" Are they actually leaked, if so then is it leaked by someone on the inside, like a producer or an engineer? Or is this oftentimes another way to make sales...some trickery maybe?

I know I could just Google all this, but
